Check Your Referral Requirements

Step 1: Confirm that we accept your insurance before booking. See insurances we are in-network with here.


Step 2: Check whether your plan requires a referral.

  • You may call your insurance company to verify, or

​

  • Simply request a referral from your primary care provider.

How to Schedule Your First Appointment

Once any referral requirements are met, call or text our office to schedule your first appointment.

  • New patient forms: These forms will be emailed to you prior to your appointment. If you are unable to complete them electronically, please print and bring the completed pages to your scheduled visit. Download forms here.

If you have an urgent referral that requires priority scheduling, please contact me directly at referrals@optimakidney.com. This is a HIPAA-compliant email address dedicated to provider communication.
